If you are recertifying for the first time and comparing continuing education options, you have probably run into two terms that get used constantly and explained rarely: CAPCE accredited and state approved. They sound like they might mean the same thing. They do not always, and understanding the difference can save you from an unpleasant surprise when you submit your hours for recertification.

What CAPCE Accreditation Actually Means

CAPCE stands for the Commission on Accreditation for Pre-Hospital Continuing Education. It is a national accrediting body, formerly known as CECBEMS, that reviews and approves EMS continuing education courses against a consistent set of educational standards. When a course carries CAPCE accreditation, it means an outside organization has evaluated the course content, instructional design, and testing methods and confirmed it meets a recognized quality bar.

CAPCE accredited courses are widely accepted by the National Registry of Emergency Medical Technicians toward NCCP recertification requirements. This national recognition is exactly why CAPCE accreditation has become the standard so many EMS continuing education providers advertise front and center.

What State-Approved CE Means

State-approved continuing education works differently. Instead of a national accrediting body reviewing the course, the individual state EMS office reviews and approves it directly, based on that state’s own specific criteria. A course can be state-approved without ever going through CAPCE, and a course can carry CAPCE accreditation without automatically being approved by every single state EMS office.

States sometimes require this direct approval because they want to confirm a course covers state-specific protocols, scope of practice differences, or regional issues that a nationally standardized CAPCE course might not address in the level of detail the state wants.

Where the Confusion Comes From

Here is the part that trips up a lot of first-time recertifiers. CAPCE accreditation is generally what satisfies the National Registry’s requirements if you are recertifying nationally. But if your state also requires its own state license, separate from your National Registry certification, that state may have its own additional continuing education requirements that a CAPCE course alone does not automatically cover.

If you also hold a separate state license, and many EMTs and paramedics do, your state may layer additional requirements on top of National Registry recertification. Some states accept CAPCE courses automatically for state purposes as well. Others require specific state-approved content in addition to your national hours, particularly around state-specific protocols or legal and scope of practice topics.

So Do You Need Both?

For most working EMTs and paramedics, the honest answer is that it depends entirely on your state. Some states have aligned their requirements closely enough with the National Registry that CAPCE-accredited education covers everything you need in one pass. Other states maintain requirements that sit outside of what CAPCE reviews, which means you may need a combination of CAPCE-accredited national and individual component hours alongside specific state-approved content for your local or state-mandated portion.

The safest approach is never to assume. Check directly with your state EMS office about what your state specifically requires, and confirm with your continuing education provider that their courses are labeled clearly for both CAPCE accreditation and any relevant state approval.
